Please Note: Please call our office if you have questions about your insurance choices. We will submit claims for many other private plans. Participation in these plans may change without notice. Our billing office number is 585.244.9841.
Annual physicals for school, sports, and camp.
Our schedule is usually available for the next few months, so please call as soon as you know your child will need a routine physical exam. We are currently using a standardized "school physical" form, accepted by all schools in Monroe County. You will receive a copy of the form and a copy of your child's immunizations at your office visit. This form is accepted for one year from the date of the exam, so you can keep the form and make copies for school, scouts, camp, and sports.
